IRB Forms Grace Period Ends Feb. 28, 2019

The grace period for using and submitting retired IRB forms (including applications and templates) will end on Thursday, Feb. 28, 2019. On Jan. 21, 2019, the IRB released revised forms based on changes to the IRB regulations. As of Feb. 28, the IRB will require the use of these new forms and will no longer accept older versions.

Any submissions after Feb. 28 containing outdated forms will be sent back to the researcher for correction. The IRB highly recommends that researchers always download new forms directly from the IRB website prior to any submission to ensure they are using the most current versions.
